Sentara Halifax Regional Hospital issued the following announcement on Dec. 21
Sentara RMH Medical Center has been recognized as a Health Quality Innovator for 2021 in the Population Health category. The Health Quality Innovator Awards is the annual awards program created by Health Quality Innovators (HQI) that recognizes and celebrates organizations using successful, evidence-based approaches to quality improvement.
Sentara RMH was selected as the winner in the Population Health category for its Diabetes Self-Management Education and Support (DSMES) program. This category recognizes organizations that work to improve the health of patients and communities through prevention, treatment, and improved access to care.
Sentara RMH enhanced their diabetes education program when data revealed a significant increase in uncontrolled diabetes and hypertension in their community. The need for this program enhancement resulted from patients not seeking health care services during the COVID-19 pandemic. In addition, the diabetes education program was adapted to reach patients from different settings and cultures. As a result, the program improved the quality of life, reduced the mortality rate and decreased hospital admissions for their patients. By 2021, this shift in focus from hospital-based diabetes education to the DSMES program, held in outpatient primary care settings, has served approximately 900 patients.
